Passport Size Photograph

Colour photograph with white background, recently taken without cap or sunglasses. Max 500 KB (JPEG, JPG, PNG).

Candidate Signature

Black or dark blue ink on plain white paper. Max 300 KB (JPEG, JPG, PNG).

Documents Required for Upload (PDF, max 500 KB):

HSLC Certificate as proof of educational qualification
Birth Certificate or HSLC Certificate as proof of age.
Voter ID Card and Aadhaar Card as proof of identity.
Residential Certificate as proof of residency.
ST/SC/OBC Certificate (whichever is applicable)
Certificate of Eligibility for Meritorious Sportsperson Category (if applicable)
Certificate of Employment for MRHG & CD Employee Category (if applicable)
Ex-Servicemen Certificate for Ex-Servicemen Category (if applicable)
Certificate of Computer knowledge (CCC or equivalent & above)
